Living Wells Community & Development Center provides Care-By-Case support for youth and families facing instability, crisis, or overwhelming circumstances. We don’t offer one-size-fits-all solutions — we walk alongside each household to create a plan that brings stability, dignity, and hope.
Care-By-Case is relationship-based, trauma-informed, and faith-rooted.

Care-By-Case is for:
- Families facing housing instability
- Youth at risk
- Caregivers who feel overwhelmed
- Families transitioning out of crisis
- Households connected to foster care, kinship care, or guardianship
You do not need to be perfect or have everything figured out.
